If you have failed Volterra units and would like us to perform a Failure Analysis (FA), please fill out the form below and return it with a few of the failures. (Please email or fax a copy of the form prior to sending to alert Quality in advance of receipt of the units.) Be sure to include as much information as possible about the failure and the conditions under which it failed. Any additional schematics, test set up and application information you can provide will aid in the FA process. Also, make sure to include your contact information so someone from the Quality Department can contact you with the results. There are instructions on the form for how to handle and send the failed units back to Volterra.
